Seaway Valley Community Health Centre (SVCHC) is a not-for-profit, community-governed organization providing primary health care, health promotion and illness prevention programs and services to residents of the Cornwall and area. In particular, we serve people experiencing barriers to accessing health care services and work in partnership with other community-based health and social service organizations to address the social determinants of health. SVCHC is a fast-paced work environment and all candidates must be able to work well independently and as part of an interdisciplinary team. All candidates must have a passion for client care within your community. SVCHC is recruiting the following three positions . We offer a competitive salary and comprehensive benefit package, including HOOPP. All positions involve working 35 hours/week, including one evening per week. Successful candidates must undergo a vulnerable sector screening to the satisfaction of the SVCHC. Respiratory Therapist Full time, Permanent, Cornwall & Area POSITION SUMMARY: The Respiratory Therapist (RT) works to provide care in accordance with the standards of professional practice as outlined by the Canadian Society of Respiratory Therapists (CSRT) to community clients living with Chronic Obstructive Pulmonary Disease (COPD) and other chronic respiratory illnesses. Care is provided through sharing of information, conducting spirometry tests, developing, implementing and revising customized self management plans, and group education sessions. The RT is responsible for establishing and maintaining strong communication pathways between clients, primary care providers, specialists, our local hospitals and other community resources. As a member of the interdisciplinary team the RT supports members of the clinical team as a lung health expert resource by providing information about Asthma, COPD, smoking cessation, etc. EDUCATION, SKILLS & QUALIFICATIONS: •Member in good standing with the College of Respiratory Therapists of Ontario •Eligible for membership with the Canadian Society of Respiratory Therapists •CRE designation •3-5 years of experience as an RT •Demonstrated skills in applying adult education principles •Current Cardiopulmonary Resuscitation (CPR), Level C •Smoking cessation ¡V TEACH trained preferable •Knowledge of the Chronic Disease Prevention Model and self-management programs •Proficiency working in a computerized environment and working with electronic client records •Must possess strong organizational, communication and interpersonal skills •Demonstrate effective time-management skills and strong decision-making skills •Ability to work independently with minimal supervision •Previous experience working with culturally diverse and low income populations. •Experience and understanding of rural communities and rural health needs. •Excellent communication skills in English (oral and written) required; French and other languages considered an asset. •Valid driver's license and access to a vehicle required. Nurse Practitioner, COPD Outreach and Lung Health Program Full Time, Permanent, Cornwall & Area Position Description/Responsibilities: The Nurse Practitioner (NP) in the Community COPD Outreach and Lung Health Program provides a wide range of primary care services including comprehensive triage, screening , physical assessment, diagnosis and treatment as well as counseling, interpretation of test results, referral and follow-up, health education and preventative care to clients with varied health, social, cultural and economic needs. The NP encourages individuals to take responsibility for their own health by involving them in risk factor and health problem identification, goal setting and the choices of interventions for disease treatment and prevention and health promotion. The NP must possess the ability to function independently and as a member of an inter-professional team in a culturally sensitive manner. EDUCATION, SKILLS & QUALIFICATIONS: •Baccalaureate in Nursing from a recognized university. •Current and valid registration in the Extended Class with the RNEC College of Nurses of Ontario. •Must possess strong diagnostic abilities, clinical and management skills of the respiratory client population. •3-5 years of experience as an NP Certified respiratory educator is preferred. •Previous critical care and emergency nursing experience an asset •Demonstrate effective time-management skills and strong decision-making skills •Must possess strong organizational, communication and interpersonal skills •Proficiency working in a computerized environment and working with electronic client records •Previous experience working with culturally diverse and low income populations. •Experience and understanding of rural communities and rural health needs. •Excellent communication skills in English (oral and written) required; French and other languages considered an asset. •Valid driver's license and access to a vehicle required. Primary Care Team Leader Full time, up to 18 month contract, Cornwall POSITION SUMMARY: The Primary Care Team Leader (Team Leader) works as part of the interdisciplinary team, responsible to oversee day-to-day clinical operational issues, coordination of clinical scheduling and supporting effective team functioning of the Primary Health Care Team. The Team Lead coordinates and monitors work flow processes to ensure efficient functioning of clinical operations. This individual participates in program development and develops and monitors the Team's orientation to and implementation of policies, procedures and protocols related to the primary care services delivered. The Team Leader provides leadership to ensure the team meets organizational objectives and provides direct client service as required. This position is responsible for coordination and implementation of training initiatives for Team members and orientation, training, supervision and guidance to clinical placement students and volunteers. The Team Leader provides training and support of the EMR and the collection of data required for reporting to the funders and the Centre and coordinates the physical environment in the clinical areas. EDUCATION, SKILLS & QUALIFICATIONS: •Bachelor Degree in an Allied Health Profession (i.e. Social Work, Registered Nurse, Therapist). •Current and unrestricted registration with the related College of Ontario. •3-5 years of clinical experience preferably working in a community setting. •Sound judgement and ability to pay close attention to detail. •Demonstrate effective time-management skills and ability to work independently with minimal supervision. Demonstrated ability to work effectively as a member of an inter-disciplinary team. •Must possess strong organizational, decision-making, communication and interpersonal skills •Proficiency working in a computerized environment and working with electronic client records •Previous experience working with culturally diverse and low income populations. •Experience and understanding of rural communities and rural health needs. •Excellent communication skills in English (oral and written) required; French and other languages considered an asset. •Valid driver's license and access to a vehicle required.
